Care Agreement

Hi — I'm John, and Pax Pals is just me: one person caring for your dog in my own Logan Square home, like they're my own. This Care Agreement keeps your pup safe and makes sure we're on the same page about health, emergencies, and the everyday risks of dog care. You agree to it when you check the box while booking, and it works alongside the Booking Terms.
It's just me
There's no staff, no kennel, and no app in between. When you book, you're trusting me personally with your dog — and I'll always treat them with the same love and attention I give my own.
Vaccinations & health
Before their first visit, your dog needs to be healthy and up to date on their core vaccinations:
- Rabies
- Distemper / parvo (the DHPP combo)
- Bordetella (kennel cough)
They should also be free of fleas and ticks and not be carrying anything contagious. Please tell me about any medical conditions, medications, or allergies up front, so I can take the best possible care of them.
Honesty about behavior
Please be straight with me about your dog's temperament, including any history of biting or aggression toward people or other animals. It keeps everyone safe — your dog included. If a dog turns out to be a risk to others, I may need to end a stay early and reach you to pick them up.
Emergencies & vet care
If your dog seems sick or hurt, I'll try to reach you and your emergency contact right away. If I can't reach anyone and your dog needs help, you authorize me to get them veterinary care — ideally from your own vet — and to approve treatment up to $500 without waiting for the okay. You're responsible for the cost of any vet or emergency care your dog needs while they're with me, and I'll keep you posted every step of the way.
Food, meds & comforts
Please send your dog's own food, any medications (labeled, with instructions), and a favorite toy or blanket if it helps them settle in. Sticking to their normal routine keeps tummies happy and tails wagging.
Photos & updates
I love sending photo updates so you can see your pup having a good time. With your okay, I might also share the occasional photo on Pax Pals social — never with your name or contact info — and you can opt out anytime, no questions asked.
My home & property
Daycare and boarding happen in my home. You're responsible for any damage or injury your dog causes beyond normal wear and tear, to the extent the law allows.
The everyday risks of dog care
Even with great care, dogs play, wrestle, and get the zoomies — and pet care carries some natural risk. By booking, you accept those everyday risks. This doesn't cover harm caused by my own negligence or misconduct.
Release
Except for harm caused by my negligence or willful misconduct, you release Pax Pals (John Khuu) from claims arising out of your dog's care, behavior, or pre-existing conditions, to the fullest extent allowed by Illinois law.
Saying yes
Checking the agreement box when you book means you've read and agreed to this Care Agreement — for yourself and anyone else who shares ownership of your dog.
